What is an Open Technical Functional Septorhinoplasty?
Open-technique functional septorhinoplasty is a method frequently used in rhinoplasty surgeries. The bone and cartilage structures in the nose are completely exposed through small incisions made in the nasal tip area. This technique allows one to observe the nasal anatomy from a wider angle and correct the problematic areas more precisely. This method provides a great advantage, especially in operations that aim to improve nasal functions and aesthetic interventions.
During the operation, a small incision is made between the nostrils in the area called the columella. This incision makes it possible to see the bone and hyaline cartilage tissues under the skin of the nose. This way, the surgeon can examine the nose’s internal structure in detail and make precise adjustments to the nose. The open technique method allows the surgeon to obtain a wider view and is especially preferred in complex cases.
Open nose aesthetics is applied in the following cases:
Correction of the shape of the nose
It is applied to ensure that the nose has a harmonious appearance with the face.
Eliminate breathing problems:
Breathing problems caused by curvatures or structural problems in the nose are corrected.
Correction of the nasal hump:
Visual problems such as an arch or hump on the back of the nose are corrected.
Elimination of nasal tip asymmetries:
Deformations or asymmetries at the tip of the can be corrected.
This method allows the surgeon to perform more precise interventions. The open technique is often preferred especially in revision surgeries performed to correct the previous problems that occurred in past nose surgeries. For the surgical process to be successful this method should be executed by an experienced Otorhinolaryngologist specialist. The patient must be carefully monitored throughout the postoperative recovery process, to prevent infections of the incisions around the nose.
What techniques are used in rhinoplasty surgery and what should be considered?
Although different surgical techniques are used in rhinoplasty surgeries, three main factors determine the success of the operation:
- Cartilage, bone structure, and skin quality of the patient: This factor directly affects how surgical techniques will be applied and the outcome of the surgery.
- Choosing the right doctor: Choosing an experienced surgeon plays a critical role in the successful outcome of the surgery.
- Used surgical technique: Modern techniques supported by technological advances ensure the surgery is safer and more effective.
What is Micromotor Rhinoplasty?
The open technique functional micromotor rhinoplasty, that I use in my practice, is a method that enables precise interventions to be made with the help of modern devices. This technique offers the opportunity to make millimetric corrections on bone and cartilage tissues with the assistance of special electrical instruments used during surgery.
Micromotor rhinoplasty bears many advantages compared to traditional methods. Especially on highly arched or wide noses, a more precise result is obtained using this technique. Unlike the use of chisel or file tools in traditional methods, micromotor devices and diamond tips are preferred in this technique. This allows the bone to be shaped deliberately while helping to reduce complications such as postoperative swelling and bruising.
Why is Precision Rhinoplasty Preferred?
Precision rhinoplasty allows the surgeon to work on bone and cartilage tissues in a careful and controlled manner. With electrical devices (such as microtur, micro-saw, and piezo), shaping can be done using the vibration method without breaking the bone. Unlike traditional surgical methods, rhinoplasty procedures become safer thanks to these devices used during precision rhinoplasty.
Advantages of precision rhinoplasty techniques:
- Soft tissues are not damaged during the shaping of bone tissues.
- Diamond inserts allow millimetric corrections to be made in the nose.
- Complications such as postoperative swelling and bruising are minimized.
- The healing process is quicker and more comfortable.
Postoperative Recovery Process and Return to Social Life
The healing process after rhinoplasty may differ depending on the skin and bone structure of the patient. While these effects are usually milder in patients with thin skin and sensitive bone structures, postoperative swelling and bruising may be more prominent in patients with thick skin and hard bone structures.
Things to be considered during the postoperative recovery process:
12. Swelling and bruising: Swelling and bruising is normal for the first few days. It is recommended for the patient to apply an ice compress.
13. Recovery time: On an average of 7-10 days, the patient can return to desk duty. However, heavy physical activities should be avoided.
14. Sports activities: Heavy sports should be postponed for 2-3 weeks after surgery.
15. Full recovery period: Usually takes 6-12 months for the nose to regain its final shape.
The stitches and tampons around the nose should be carefully checked in the first week after the rhinoplasty surgery. Regular use of antiseptic solutions and antibiotics is recommended to the patient to prevent the risk of infection. The healing process may vary depending on the patient’s nasal structure; therefore, patients need to focus on their own process and not compare themselves with others.
Personalized Nose Aesthetics: Our Goals and Approach
Our main goal in personalized rhinoplasty is to achieve a natural appearance compatible with the patient’s expectations and facial structure. In this process, the trust in the relationship between the surgeon and the patient is of great importance. For the successful outcome of the operation, one or two face-to-face meetings with patients before the operation is a critical step.
The patient’s expectations from rhinoplasty are evaluated in detail during these interviews. Some patients may not be able to fully envision the change they will experience after rhinoplasty surgery. Therefore, patients need to adopt the new nose structure psychologically before surgery.
Customised Solutions
Instead of a standard design in rhinoplasty, personal solutions are developed for each patient’s facial structure. For example:
– A small curved nose may not look natural on a patient with large and long facial features. In such cases, a nose compatible with the general contours of the face is designed.
– In patients with thick skin and a wide nasal tip, additional surgical interventions may be required to narrow the nasal tip and achieve a thin appearance.
Planning these details before surgery makes it easier for the patient to achieve their desired results after the operation. We aim to design natural-looking noses that are suitable for the facial anatomy of each individual instead of applying the same nose structure to each patient.
Effects of a Successful Rhinoplasty
A successful rhinoplasty positively affects not only the physical appearance but also the psychological state and self-confidence of the individual. By providing perfect harmony with facial features well-designed nose draws attention to the eyes, cheekbones, and center of the face.
Patients are usually surprised and pleased to see how their facial features change after surgery. This change positively contributes to their social life and the way they express themselves. My goal as a surgeon is to see the satisfaction on my patients’ faces and to be happy to observe this change.
